IMPORTANT NOTES
for filing Form P19

1.	Documentary evidence are NOT required to be filed in the following cases:

	a.	Assignment – if the Form P19 is signed by or on behalf of the assignor.
	b.	Mortgage, licence/sub-licence or security – if the Form P19 is signed by or on behalf of the mortgagor, the grantor of the licence or security.

2.	If the Form P19 is signed by or on behalf of the assignor, mortgagor or grantor of the licence or security as the case may be, please indicate the same (e.g. agent of the assignor) in the field “Name and Official Capacity of Signatory” of Part 10 of the Form P19.
