For e-filing of Form D6, D7 or D9 by way of the “General Form GE” (Web-Form GE)

1. Download the pdf version of the Form D6, D7 or D9 at “Forms and Fees”
2. Fill in the Form D6, D7 or D9 and save the completed form as a pdf file
3. Go to the E-Filing System and create a Web-Form GE
4. Fill in the Web-Form GE and upload the completed Form D6, D7 or D9 (pdf file) in Part 02 of the Web-Form GE
5. Finalize and submit the Web-Form GE
